By accepting these Terms and Conditions,

• Parent agrees to pay 9 installment payments from date of purchase.
• Parent confirms his/her acceptance that his/her child has no limitations, injuries or conditions that would affect said child’s ability to participate safely in this program.
• Parent confirms his/her acceptance that his/her child is of age to participate in the Endeavor Plus program.
• Parent confirms his/her acceptance that the School Leader reserves the right, at his or her discretion, to withdraw any Endeavor Plus participant whose influence or actions are deemed unsatisfactory, dangerous or incompatible to the school community or who will not live within the rules and policies of the school. If this occurs, no reduction or return of fee, or any part thereof will be made. Parents will also be responsible for any extra costs associated with returning their child home early.
• Parent confirms his/her acceptance that his/her child may have their photograph and/or video taken and such material can be used by Endeavor Plus and/or Endeavor School’s business partners for promotion or marketing purposes.

Cancellation Policy:

• Refunds and/or make-up sessions will not be made for non-attendance for a scheduled session.
• Immediate refunds are only granted if the child is no longer enrolled at the school or has a medical note from his/her physician lasting 30 days.
• Refunds and/or make-up sessions will not be made for school-related absences, such as field trips, vacation days, in-service days, photo days or other similar instances.

Waiver of Liability:

• In consideration of being permitted to attend Endeavor Plus extracurricular activities, on behalf of myself, my family, my heirs, and my assigns, I hereby release Endeavor Plus, LLC, its agents, and its employees from liability for injury, loss, or death to the registered participant while using any facility or equipment or in any way associated with participating in the activities Endeavor Plus offers now or in the future.
• I have read the preceding information and do not need further clarification. I know, understand, and appreciate the risks associated with participation in the activity and I am voluntarily enrolling my child in the activity. Endeavor Plus takes every measure to always create and maintain the safest environment possible; however, I acknowledge that the participant is assuming all of the inherent risks of the activity. I further understand that in the event of a medical emergency, Endeavor Plus will call EMS to render assistance and that I will be financially responsible for any expenses involved.
• I accept these conditions for my child’s participation in the Endeavor Plus program.
